Down the microscope

Case: ‘Boof’ - 8yo, male neutered, Golden Retriever

Owner reports a slowly enlarging submandibular mass. On physical exam a mass approximately 3cm in diameter was noted in the submandibular region. In-clinic CBC (ProCyte Dx®) revealed a mild lymphocytosis – 5.6 x 109/L (0.9-3.5 x 109/L).
Fine needle aspirate revealed a predominance of small to intermediate-sized lymphocytes – nuclei 1.25 to 1.5 x RBC – which often have cytoplasmic extensions/tails.

Diagnosis: T-zone lymphoma. A common variant of indolent lymphoma in dogs – excision is often curative, usually a single lymph node in head/neck region. Concurrent lymphocytosis in up to 64% of cases with circulating neoplastic cells. ‘Hand mirror’ cells or uropodia on smeared cytology preparations very suggestive.

Histology: Diffuse expansion of paracortical zone with compression of remnant follicles against capsule – CD3 positive cells on Immunohistochemistry
